This Legal Form is to be completed by ALL applicants for ANY ministry position, volunteer or compensated. This is an important part of our screening process which is used to facilitate a safe and secure environment for those who participate in our programs of ministry. This Legal Form must be submitted BEFORE your participation in any ministry.

Please Note: Your answers in the “Personal Questions” section do not necessarily mean that you will be denied involvement in the ministries program at Cedarville University. It may mean that appropriate and reasonable accommodations will be required to fit your circumstances. Statement of Agreement
This Agreement and Release is made on this day by and between The Cedarville University, an Ohio Corporation, not for profit (University) and me.

RECITALS

A. Team Member now is or will be voluntarily participating in an activity, and traveling to and from said activity, which is supervised, arranged, or sponsored by or in some other manner involves the University. University involvement may vary from close supervision, if any, to incidental contact between the University and Team Member.
B. For the purposes of this Agreement and Release, the Team Member may be engaged in study, with or without University credit, participating in an internship, ministry, or other activity designed to prepare him or her for occupational ability and/or representation of the University in sports, music or speaking engagements.

COVENANTS
In consideration of the University's Agreement to allow Team Member to participate or, as the case may be, to continue to participate in an activity included in the description in paragraph A. above, Team Member promises as follows:

1. The Team Member, at his/her own cost, shall arrange for and maintain life, health, major medical and hospitalization insurance during the period of such activities. Such insurance shall provide coverage for any and all expenses caused by illness, injury, accident or death during the period of the activity aforementioned.
2. In consideration of the opportunity to engage in such activity, the Team Member hereby and for his or her heirs, executors, administrators, successors and assigns, assumes the risk and RELEASES, forever discharges and holds the University harmless from any and all claims whatsoever resulting from illness, accident, injury, hospitalization or death of the Team Member arising from any such activity.
3. In addition, the Team Member further RELEASES, holds harmless and forever discharges any agents, employees, successors, administrators and all other persons who may be acting on behalf of or in the stead of the University and with respect to all activities contemplated herein.
4. The Team Member hereby consents to the use of any images, pictures, videos, and the like for purposes of promotion, recruitment, reporting, or similar disclosures of the team's activities. The undersigned further declare(s) and represent(s) that no promise, inducement or agreement not set forth herein, has been made to the undersigned, and that this Release contains the entire agreement between the parties hereto. THE UNDERSIGNED HAS READ THE FOREGOING RELEASE AND FULLY UNDERSTANDS IT.
The information contained in this form is correct to the best of my knowledge and any false information may result in my being discontinued from the ministry. I authorize any references listed in this form to give Cedarville University any information they may have regarding my character and fitness for volunteer or compensated service. I release all such sources from liability for any damage that may result from furnishing such evaluations to Cedarville University, and I waive any right that I may have to access references provided on my behalf. Cedarville University reserves the right to make final decisions in regards to program participation. I agree to be bound by the guidelines and policies of Cedarville University, and to refrain from unscriptural conduct in the performance of my services in any church or organization.*
